Sometimes custom properties do not update

We use to transition our files to an "under review" state, during the transitions some variable are updated to match the file revision letter and increment an internal numeric revision variable by +1 with a dispatch.

Let's say it works >99% of times, but for unknown reasons there are those rare exceptions popping up heere and there in which the custom properties are not automatically updated. In the file history I can see the comment left by the dispatch triggered upon the transition, but the corresponding variable in the 3d model is untouched. I also looked at the workstation PDM admin logs and the client does not exibit errors.
So I tried to rollback the files and trigger the transition again using the same user, but logged in my workstation and using windows explorer: everything is moving as expected.
